Alicia had thirty-one browser tabs open and a notebook with three crossed-out phone numbers. Her younger brother had finally said yes to getting help, and the window was small. She’d been told, by more than one person, that he might change his mind by Friday.
Every program she found said the right things. Compassionate. Evidence-based. Individualized. She couldn’t tell the difference between them. “They all sound like they were written by the same person,” she told her husband at midnight. “How do I know which one is actually good?”
If you’re in a similar place, with a decision to make quickly and a lot of polished language to wade through, this guide is for you. It won’t tell you which program to choose. It will give you better questions to ask.

Why the Brochure Doesn’t Tell You Enough
Marketing language is designed to reassure, and almost every provider does it. That doesn’t make any individual program untrustworthy. It just means the words on a website are a poor way to compare. The comparison comes from specifics: who works there, how decisions are made, what happens when something goes wrong, and what the plan is after discharge.
Two published guides, one from the American Institutes for Research and one from the National Association for Children of Addiction, give helpful checklists. Neither is a ranking, and neither names a “best” program. They simply tell you what a thoughtful person would want to know.
Start With Accreditation and Staff
Both guides begin in the same place: who is accountable, and who is doing the work.
- Accreditation. The American Institutes for Research guide suggests asking whether the program is accredited by the Joint Commission or CARF. The NACoA guide asks the same, along with whether the center is state-licensed
- Licensed staff. Ask how many staff are licensed and credentialed, and how many are not
- A mixed team. The AIR guide suggests checking for a range of professionals, including addiction and mental health counselors, a medical doctor, a nurse, a psychiatrist and a peer counselor
- Staff ratios. In the AIR guide’s words, ratios “should be low enough to ensure each person can see a counselor when they need to”
You don’t need to understand every credential. You need to hear confident, specific answers. Hesitation or vagueness is information too.
Ask How They Assess and Plan
Treatment shouldn’t begin with a generic template. The NACoA guide asks whether admission includes a thorough assessment by licensed professionals, and whether co-occurring mental health conditions are screened. The AIR guide similarly says staff should evaluate physical and mental health needs and living situation to recommend the right level of care.
A useful follow-up is: “How would the plan change if something doesn’t fit?” A good program has an answer.
If a medication is likely to be part of care, ask about it directly. The AIR guide notes that people who take medication for opioid use disorder are more likely to stay in treatment, and suggests asking whether the program provides it or can connect you with someone who does. Whether it’s right for any individual is a decision for their doctor.
Ask About the Structure of the Day and the Stay
The NACoA guide suggests asking about the average length of stay, how discharge readiness is decided, the size of the program, and what a typical daily schedule looks like. These questions help you picture the experience, and they reveal how much thought has gone into it.
It also suggests asking about crisis protocols: what happens if there’s a relapse or an emergency during treatment, and whether there is 24/7 coverage.
Ask About Family and Aftercare
Many people find that the biggest gap isn’t treatment itself. It’s the weeks after. Both guides point toward asking:
- Is there a family program, and who can take part?
- How are families included in planning and continuing care?
- What support exists after discharge, such as outpatient therapy, alumni groups or peer support?
- What does aftercare cost?
Alicia realized she hadn’t asked a single question about the day her brother would leave.
A Familiar Scenario: What Alicia Did
She stopped scrolling and wrote one page. Ten questions, drawn from the checklists. Then she made calls, and she wrote down not just the answers but the way they were given.
One program answered every question quickly and smoothly, and then pivoted to talk about price. Another paused on a question about aftercare and said, “Honestly, that’s an area we’re building.” A third gave her the clinical director’s name and offered to have her call back. She didn’t pick the most polished one. She picked the one that was straightforward about what it didn’t have yet.
Reading Provider Articles With a Careful Eye
As you research, you’ll read a lot of articles written by treatment providers. They can be useful, as long as you remember they’re also a way of introducing the provider. For example, Renewal Springs, a detox center in Oklahoma City, publishes an explainer on detox complications when meth and fentanyl are used together. It’s a good example of how a provider explains a complicated situation, and a reminder to bring your own questions to the call: what would happen in our case, and who would be responsible for what?
